I use the little plastic aftermarket clips. The ones I bought come in a two-pack (I bought two packs, so four clips) from, oh, one of those schtuff catalogues like Carol Wright Gifts and their ilk. Or maybe it was Dr. Leonard. Anyway, the clip makes it more comfortable and they're very inexpensive.
